Request Vital Records

Vital records:  birth, death or marriage records.

Birth records are kept in the following locations: The municipality the child was born in and the municipality that the mother was living in at the time of the birth.

Marriage records are kept in the following locations: The residence municipality at the time of marriage. 

Death records are kept in the following locations: The residence municipality and the municipality that the death occurred.

Non-Certified copies of vital records are available at a rate of $5 per copy. Non certified copies are for records information only and not to be used for legal purposes.

Certified copies of vital records are available at a rate of $15 for the first copy, and $6 for each additional copy ordered during the same transaction. You may obtain a certified copy in one of three ways:

Visit the Maine Municipal Vital Record Request Service to order a vital record online.

Order by Mail

To order a vital record by mail, please complete this request form and mail it to Westbrook City Clerk's Office, 2 York Street, Westbrook, Maine 04092.

Additional Information
